Oak Hill is recruiting a Direct Support Professional in Oxford, CT. The role involves supporting daily living activities, health and wellness, community participation, nutrition, mobility, and detailed documentation. The position is union, non-exempt, with 40 weekly hours and overtime eligibility.

Training provided; benefits offered. Candidate must be 18+, have a high school diploma or GED, valid driver’s license, and clear background checks.

Qualifications

• Must be at least 18 years of age.
• Valid driver’s license with the ability to transport individuals in agency vehicles.
• Ability to complete required background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain necessary certifications.

Responsibilities

• Assist with daily living activities including personal care, grooming, bathing, dressing and hygiene as per support plans.
• Monitor health information and assist with health care tasks as required, including medication administration per directives.
• Transport individuals to appointments and community activities using agency vehicles.
• Prepare meals and snacks per menus and dietary needs; provide mealtime support as needed.
• Assist with mobility and transfers using approved techniques and equipment.
• Document daily activities, health information, progress notes and medication administration in the EHR; follow regulatory requirements.
• Respond to emergencies with established procedures and document related activities; apply First Aid/CPR when needed.
• Collaborate with team members; participate in required trainings and meetings.
