Qualifications

• Must be at least 18 years old.
• High school diploma or GED.
• Valid driver’s license to transport individuals in agency vehicles.
• Ability to pass background checks, fingerprinting, and pre-employment health screenings.
• Commitment to safety and professional conduct per Oak Hill policies.
• Willingness to stay beyond scheduled shifts to ensure coverage.

Responsibilities

• Support daily living activities and personal care per plans.
• Monitor health information and document vitals when required.
• Administer medications following training and directives.
• Transport individuals to appointments and activities using agency vehicles.
• Prepare meals according to menus and dietary needs.
• Assist with mobility, transfers, and equipment per safety protocols.
• Document activities, health data, and medications in the EHR.
• Respond to emergencies using established procedures.
