Job summary

Oak Hill in Granby, CT is hiring a Direct Support Professional to assist individuals with daily living, health monitoring, and community participation. The role requires compassionate care, documentation in the EHR, and adherence to health and safety policies.

No prior experience required; paid training is provided and opportunities for career growth. You'll transport clients, assist with meals, mobility, and medication administration, and work a 32-hour weekly schedule Friday through Thursday

Qualifications

• Must be at least 18 years of age.
• High school diploma or GED.
• Valid driver's license with the ability to transport individuals in agency vehicles.
• Ability to successfully complete required background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain necessary certifications.
• Commitment to maintaining a safe, healthy, and professional work environment in accordance with Oak Hill policies.
• Commitment to continuity of care, including an occasional need to stay beyond a scheduled shift until coverage is in place.

Responsibilities

• Assist with daily living activities including personal care, grooming, bathing, dressing, and hygiene needs.
• Monitor health and document vital signs when required.
• Administer medications and maintain accurate medication records.
• Transport individuals to appointments and activities using agency vehicles.
• Prepare meals and snacks according to menus and dietary requirements.
• Document daily activities, health information, and incidents in the EHR.
• Respond to emergencies and apply First Aid/CPR when required.
• Collaborate with team and participate in meetings and trainings.
