Job summary

REM Community Services, a part of the Sevita family, provides community-based services for individuals with intellectual and developmental disabilities. The position involves direct support in residential settings, assisting with daily living activities, health maintenance, transportation, and community integration. A valid driver’s license and clean MVR are required.

Schedule: Monday–Thursday 7:30AM–3PM and alternating weekends. Wage: $16.68/hour.

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Valid driver’s license in good standing; car registration if providing transportation.

Responsibilities

• Directly support individuals in residential programs per service plans.
• Document progress and maintain confidentiality of records.
• Manage purchases within the individual’s budget and preferences.
• Maintain relationships with individuals, families, and case managers; follow customer service standards.
• Report abuse, neglect, and incidents per policy and external requirements.
• Assist with daily living activities including health maintenance, transportation, and community integration.
• Provide health care support such as medication administration if assigned; relay information to nurses or supervisors.
• Assist with meal preparation and monitoring of dietary plans.
