Job summary

Hope Network / Bay Valley House in Bay City, MI seeks a Direct Support Professional (DSP) for 1st shift. This full-time, on-site role supports individuals to achieve independence, assisting with activities of daily living, community participation, and health management.

You will monitor safety, communicate effectively, and collaborate with a care team to implement care plans. Candidates should have a high school diploma or GED, a Michigan driver’s license, and be able to lift 50 lbs.

Qualifications

• High school diploma or GED required.
• Valid Michigan driver’s license.
• Ability to lift 50 lbs.
• CPR/First Aid certification preferred or required.
• Shift availability includes weekends and holidays.
• Basic computer skills.

Responsibilities

• Monitor and ensure safety 24/7 with 15- or 30-minute checks as needed.
• Support crisis prevention, intervention, and resolution.
• Build trust through consistent, meaningful communication.
• Foster partnerships with individuals served, community, and professionals.
• Help develop problem-solving, communication, and independence skills.
• Assist with social, leisure, and community activities.
• Assist with activities of daily living (getting to places, dressing, bathing, grooming, nails, restroom, cooking).
• Monitor vitals for medical conditions and relay information to care team.
• Promote positive behavior and support medical/clinical care plans.
• Document accurately and timely in care plans and records.
• Connect individuals with community opportunities and employment or education goals.
