Job summary

Independent Case Management Inc. seeks compassionate aides to provide daily personal care, companionship, transportation coordination and home support.

You will assist with activities of daily living and medications, document updates on an online platform, and ensure safety through constant observation and support. Applicants must have a high school diploma or equivalent, adhere to HIPAA and SOPs, maintain attendance, and follow agency policies.

Qualifications

Responsibilities

• Provide daily care activities such as bathing, bathroom assistance, feeding, menu planning, dressing and grooming; assist with medication and treatments as prescribed.
• Assist with scheduling, arranging transportation, and accompanying customers to appointments and errands as needed.
• Support customers to achieve agreed goals to improve quality of life and community integration.
• Provide written daily updates of each shift’s activities using an online platform.
• Monitor customer condition and surroundings to ensure safety and well-being, including equipment and supply checks.
• Perform other duties as assigned; demonstrate quality decision making, effective communication and problem solving.
• Maintain a positive work ethic and be receptive to feedback while collaborating with the care team.

Job description

• Providing quality care, direction, guidance, and help to customers with daily activities, such as bathing and bathroomfunctions, feeding, planning menus, dressing, grooming, taking medication and/or administering treatments, and housework.
• Help customers make and keep appointments, provide or arrange transportation to run errands and serve as a companion
• Help customer achieve agreed-upon goals to improve quality of life and integration into the community
• Provide written daily updates of each shift’s activities using an online platform
• Observe the customer’s condition and monitor the surroundings to ensure customer safety and success. This includes monitoring of equipment and materials
