Job summary

Qualifications

• Must have a High School Diploma or GED and be at least 18.
• Possess a valid driver's license and reliable transportation.
• Be comfortable with technology and able to complete documentation.
• Background checks and health screening (TB) required.

Responsibilities

• Assist individuals with goal completion per the individualized support plan.
• Provide ADL support (bathing, grooming, meals, etc.) per needs.
• Safely transport individuals in company or personal vehicles.
• Accompany individuals to community activities and events.
• Complete documentation in the EHR promptly.
• Engage with individuals in home and community.
• Uphold rights and maintain confidentiality under HIPAA.
• Maintain professional boundaries and ethical behavior.
• Serve as a mandated reporter for abuse or neglect.
• Administer prescribed medication after training.
• Remain awake and alert during shifts and perform physical duties as needed.
• Adhere to company policies and values.
