Job summary

00 RHA Health Services, LLC seeks a Direct Support Mentor to provide mentorship, coaching, and leadership for Direct Support staff in residential and day settings. The role supports people with IDD, mental health, and substance use challenges, and may involve sleep-over duties on certain shifts.

Responsibilities include assisting with personal outcomes, collaborating with the interdisciplinary team, promoting dignity and choice, and ensuring safe, clean living environments.

Qualifications

• Must have a High school diploma or GED.
• Valid driver's license with valid personal auto insurance is required.
• CPR and First Aid certification, Med Tech and NCI certifications may be required by regulation.

Responsibilities

• Assist people supported to identify personal outcomes aligned with their goals and preferences.
• Support the Interdisciplinary Team to identify needed supports for outcomes.
• Promote dignity, respect, and quality of life for people supported.
• Provide opportunities for choice and assist with decision-making by people served.
• Ensure safety by reporting incidents of abuse, neglect, or exploitation immediately.
• Maintain cleanliness of the group home and vehicles; handle basic maintenance requests.
