Job summary

Seven Hills Community Services is seeking a Direct Support Professional II to empower individuals to live meaningful, independent lives in their communities. The role focuses on daily care, teaching life skills, and implementing ISP and Behavior Support Plans within residential settings.

You will support personal care, medication administration, and health needs, accompany appointments, and contribute to accurate data collection and mentorship of staff.

Qualifications

• Requires High School Diploma or GED.
• Active US driver’s license.
• Must be at least 18 years of age.

Responsibilities

• Provide daily support and care to individuals in a residential setting.
• Teach life skills to promote independence at home and in the community.
• Follow ISP and Behavior Support Plans; assist with personal care and medication administration.
• Support participation in community activities and health appointments; maintain documentation.
• Lead by training and mentoring other staff; assist with budgeting and managing personal funds.
• Build positive relationships with families, guardians, and team members.
