Job summary

Seven Hills Community Services is seeking a Direct Support Professional II to empower individuals to live independently in their communities. You will provide daily care, support personal development, and help implement ISP and Behavior Support Plans in residential settings.

Ideal candidates are compassionate, dependable, and ready to take on a leadership role by mentoring staff and supporting budgeting, shopping, and personal funds management.

Qualifications

• Strong problem-solving and adaptability in diverse settings.

Responsibilities

• Provide daily support and care to individuals in a residential setting.
• Teach life skills to promote independence at home and in the community.
• Follow Individual Support Plans (ISP) and Behavior Support Plans.
• Assist with personal care, medication administration and health-related needs.
• Help individuals participate in community activities and maintain social connections.
• Accompany individuals to appointments and support overall health and well-being.
