Job summary

Seven Hills Community Services is seeking a Direct Support Professional II to empower individuals to lead independent lives in their communities. You’ll provide daily support in residential settings, teach life skills, implement ISP and Behavior Support Plans, and help with personal care and medications.

You will also train and mentor staff, assist with budgeting, build strong relationships with families, and maintain a clean, welcoming home environment while upholding dignity and respect for

Responsibilities

• Provide daily support and care to individuals in a residential setting.
• Teach life skills at home and in the community to build independence.
• Follow ISP and Behavior Support Plans.
• Assist with personal care, medication administration, and health-related needs.
• Support individuals in community activities, relationships, and social connections.
• Accompany individuals to appointments and ensure health and well-being.
• Maintain accurate documentation and data collection to track progress.
• Take a leadership role by training and mentoring other staff.
• Assist with budgeting, shopping, and managing personal funds.
• Build strong relationships with families, guardians, and team members.
• Help maintain a clean, safe, and welcoming home environment.
