Job summary

Seven Hills Community Services is seeking a Direct Support Professional II to empower individuals to live meaningful, independent lives in their communities. This role emphasizes care with a leadership focus and teamwork within a mission-driven team.

You will provide daily support, teach life skills, help with personal care and medications, and promote health and well-being in residential settings. A strong emphasis on documentation, budgets, and relationships with families completes the profile.

Qualifications

• High School Diploma/GED/HiSET required.
• Active US driver’s license issued from your state of residence.
• Must be at least 18 years old.
• Strong communication, teamwork, and problem-solving skills.

Responsibilities

• Provide daily support and care to individuals in a residential setting.
• Teach life skills to promote independence at home and in the community.
• Follow ISP and Behavior Support Plans.
• Assist with personal care and medication administration.
• Support individuals in community activities and social connections.
• Accompany individuals to appointments and ensure health and well-being.
• Maintain accurate documentation and data collection to track progress.
• Take a leadership role by training and mentoring other staff.
• Assist with budgeting and managing personal funds.
• Build strong relationships with families, guardians, and team members.
• Help maintain a clean, safe, and welcoming home environment.
