Seven Hills Community Services is seeking a Direct Support Professional II to empower individuals to live independently in their communities. You will provide daily care, support personal goals, and help implement ISP and Behavior Support Plans.

Role includes supervising staff, teaching life skills, and coordinating with families. Requires a high school diploma and a valid driver's license; extensive community support essential.

Qualifications

• High School Diploma or GED required.
• Active US driver’s license from state of residence.
• Minimum age 18.
• Ability to document progress and outcomes clearly.

Responsibilities

• Provide daily support and care to individuals in residential settings.
• Teach life skills at home and in the community to foster independence.
• Follow ISPs and Behavior Support Plans.
• Assist with personal care and medication administration as needed.
• Support community activities and relationships for clients.
• Lead and mentor other staff as needed.
• Maintain a clean, safe home environment and accurate records.
