Job summary

Seven Hills Community Services seeks a Direct Support Professional II to empower individuals to live independently in their communities. You will provide daily care, teach life skills, and support personal health needs within residential settings.

You will implement ISP/Behavior Support Plans, accompany clients to appointments, document progress, and mentor other staff. A HS diploma and valid US driver’s license are required; 18+ years old.

Qualifications

• Must have strong communication skills and be able to work effectively in a team.
• Able to solve problems and adapt to changing needs in a residential setting.
• High school diploma or equivalent is required.

Responsibilities

• Provide daily support and care to individuals in a residential setting.
• Teach life skills at home and in the community to promote independence.
• Follow and implement Individual Support Plans (ISP) and Behavior Support Plans.
• Assist with personal care, medication administration, and health-related needs.
• Support individuals in community activities and social connections.
• Lead and mentor other staff, as needed, and assist with budgeting and personal funds.
