Seven Hills Community Services is seeking a Direct Support Professional II to empower individuals to live independently in their communities. You will provide daily care, help build life skills, and follow ISP/Behavior Support Plans in a residential setting.

Qualifications

• Prior experience in direct care or caregiving is preferred.
• Ability to follow Individual Support Plans (ISP) and Behavior Support Plans.
• Excellent communication and interpersonal skills for teamwork.
• Willingness to work awake shifts and on weekends as required.

Responsibilities

• Provide daily support and care to individuals in a residential setting.
• Teach life skills to promote independence in home and community.
• Assist with personal care, medication administration, and health needs.
• Support individuals in community activities and social connections.
• Maintain documentation and track progress toward goals.
• Train and mentor other staff to share best practices.
• Assist with budgeting, shopping, and managing personal funds.
• Build strong relationships with families and team members.
• Maintain a clean, safe, and welcoming home environment.
