Job summary

REM Community Services, a part of Sevita, is hiring Direct Support Professionals to assist individuals with intellectual and developmental disabilities in residential settings.

Provide direct support per individual plans, help with daily living activities, monitor health needs, document progress, and maintain confidentiality; a background in human services and a valid driver’s license are preferred.

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Responsibilities

• Direct Support: provide assistance per individual service plans.
• Records: document progress and confidentiality.
• Management of Individuals’ Assets: budget-conscious purchases.
• Relationships: maintain professional relationships with individuals, families and guardians.
• Reporting: report abuse, medical, behavioral incidents as per policy.
• Rights: respect residents’ rights and safety procedures.
• Appointments: accompany to medical visits as needed.
• Medications: assist or support medication administration when assigned.

Education

High school diploma or equivalent

Six months of experience in human services

Must be 18 years of age

Working knowledge of computers
