Direct Support Professionals assist with daily living, health maintenance, and community integration, while maintaining records and ensuring safety.

Qualifications

High school diploma or equivalent preferred.
Six months of experience in human services preferred.
Must be 18 years of age.
Working knowledge of computers.

Responsibilities

Direct Support: provide assistance per service plans including socialization, health maintenance, medication administration, and development.
Records: accurately document progress and activity; review logs and maintain confidentiality.
Management of Individuals’ Assets: make cost-effective purchases within the individual’s budget and preferences.
Relationships: maintain professional relationships with individuals, families, guardians and case managers; adhere to Sevita customer service standards.
Reporting: report abuse/neglect and medical or behavioral incidents per Sevita policy and external requirements.
Rights: protect confidentiality and rights of persons served; practice universal precautions.
