At the start of your evening, night, or weekend shift, you'll confirm the individual's current service plan, review recent notes, and check for any updates to routines or appointments. Throughout the shift, you'll follow the established schedule while allowing space for the individual to practice skills, make informed choices, and work toward personal goals at a steady pace. You may assist with transportation planning when needed, monitor safety and comfort, and keep interactions calm, respectful, and clear. To close out, you'll summarize key events and complete timely documentation so the next team member can transition smoothly.
