Responsibilities

• Assess patients' psychosocial status and contribute to care planning with the interdisciplinary team.
• Provide supportive counseling to patients and families and education about transplant options.
• Assist with referrals to community resources and coordination with Insurance Coordinators as needed.
• Participate in discharge planning and discuss advance directives with patients and families when appropriate.
• Contribute to quality improvement activities and documentation as required.
