Job summary

The role requires a Master of Social Work and a state license, with 2–5 years of related healthcare experience.

Qualifications

• Masters in Social Work required.
• Licensed to practice social work in the applicable state.
• 2–5 years of related experience in healthcare social work preferred.

Responsibilities

• Assess patients' psychosocial status and needs as part of care planning.
• Provide counseling and support to patients and families to improve quality of life.
• Coordinate with interdisciplinary team to facilitate rehab, treatment goals, and access to community resources.
• Educate patients about treatment options, advance directives, and available support services.
• Assist with referrals and linkages to vocational rehab, home health, and other services.
