Job summary

Qualifications

Responsibilities

• Assess psychosocial status and develop care plans with the patient.
• Provide counseling and education to support treatment goals.
• Support care planning and coordination with the interdisciplinary team.
• Assist with referrals to community resources and services.
• Participate in quality reviews and adapt interventions as needed.
• Coordinate patient education on treatment options and advance directives.
• Adhere to policy requirements and document MSW interventions accurately.
• Contribute to ongoing staff training and team huddles.
• Support discharge planning and continuity of care.
