Job summary

U.S. Renal Care seeks a Licensed Master Social Worker for our Monterey dialysis clinic to provide psychosocial care to patients and families, promote adjustment, and support efficient care planning and outcomes.

Candidates must have a CSWE-accredited MSW with current licensure; LCSW preferred. Strong communication and computer skills (Microsoft Office and clinical applications) are required. This is a full-time, on-site role with competitive benefits.

Qualifications

• Master's Degree in Social Work accredited by the Council on Social Work Education (CSWE)
• Current licensure in applicable state is required
• LCSW preferred
• Previous experience in dialysis social work preferred

Responsibilities

• Provide social services to dialysis patients and families
• Identify social issues and counsel patients and families
• Coordinate patient assessments and care planning with interdisciplinary team
• Attend QAPI activities and maintain community relationships
