U.S. Renal Care, Inc. is hiring a Dialysis Social Worker in Monterey, California, to deliver timely psychosocial services to dialysis patients and their families. Requires MSW experience; ASW/ACSW or LCSW preferred. In-person interview times are listed for the hosting location at 2066 N Fremont, Monterey.

Responsibilities include patient advocacy, care planning, education, and collaboration with the Interdisciplinary team to meet quality patient care and program goals.

Qualifications

• Must have MSW experience in dialysis settings.
• ASW/ACSW preferred; LCSW preferred.
• Proficient in Microsoft Office and USRC clinical applications.

Responsabilités

• Provide social services to dialysis patients and families.
• Identify psychosocial needs and counsel patients and families.
• Coordinate care planning with Interdisciplinary team.
• Support patient education and program goals.
