Job summary

U.S. Renal Care is seeking a Licensed Master Social Worker to join our team in a part-time role (20 hours weekly). This position focuses on delivering social services to patients and families to promote psychosocial adjustment to dialysis and rehabilitation.

You will participate in the interdisciplinary team, conduct initial assessments, coordinate referrals, and support program goals while complying with state regulations and company policy.

Qualifications

• Master's Degree in Social Work required; CSWE accreditation preferred.
• Current licensure in the applicable state is required or allowed per state policy.
• Experience providing social services to dialysis patients is preferred.
• Ability to communicate effectively in English, both verbally and in writing.
• Basic computer skills and proficiency with Microsoft Office and clinical software.

Responsibilities

• Provide social services to patients and families to promote psychosocial adjustment to dialysis/rehabilitation.
• Complete initial assessment of new patient psychosocial needs in accordance with state/CMS regulations.
• Participate as a member of the Interdisciplinary team in patient assessment and care planning.
• Identify social agencies and resources; coordinate referrals and supports.
• Attend and participate in required staff meetings and in-service trainings.
