Job summary

U.S. Renal Care is seeking a Licensed Master Social Worker to provide social services to patients and families, promoting psychosocial adjustment to dialysis and rehabilitation. You will support care planning and coordinate resources within a diverse team.

Role requires CSWE-accredited MSW and state licensure, with LCSW preferred; experience with dialysis patients is an asset; strong English and computer skills essential.

Qualifications

  • Master's degree in Social Work (CSWE-accredited) and LCSW preferred.
  • Current licensure in the applicable state is required unless employed in AZ, PA or Guam.
  • Experience providing social services to dialysis patients is preferred.
  • Proficiency in English (written and spoken).
  • Basic computer skills and ability to use MS Office; proficiency in USRC clinical applications within 90 days.

Responsibilities

  • Provide social services to patients and families to support social functioning and adjustment to dialysis.
  • Assist with clinical and operational processes to improve patient health and minimize hospitalizations.
  • Coordinate resources and referrals to meet patient needs and program goals.
  • Participate as a member of the interdisciplinary team in patient assessment and care planning.
  • Educate patients and families about available resources and education materials.
  • Maintain compliance with applicable state and federal laws and USRC policies.
