Job summary

U.S. Renal Care, Inc. is seeking a Licensed Master Social Worker to provide psychosocial services for dialysis patients and their families in a Michigan clinic.

The role focuses on assessing needs, counseling, and coordinating resources while supporting quality patient care and treatment adherence. The LMSW will participate in an interdisciplinary team and may engage in staff training and program development.

Qualifications

• Must have a Master’s Degree in Social Work accredited by the CSWE and LCSW preferred.
• Current licensure in applicable state is required unless employed in AZ/PA/Guam; license must be current and in good standing.
• Previous experience in providing social services to dialysis patients preferred.
• Demonstrated working knowledge of the English language and ability to communicate verbally and in writing.
• Must have basic computer skills, including MS Office; proficiency in USRC clinical applications within 90 days of hire.
• Must meet any practice requirements for the applicable state.

Responsibilities

• Assist with clinical and operational processes to improve patient health and minimize hospitalizations and missed treatments.
• Coordinate transient arrangements.
• Demonstrate effective use of supplies.
• Perform duties as assigned to meet patient care or operational needs of the clinic.
• Identify and counsel psychosocial issues.
• Provide patient and family education.
• Assist with program's target goals for patient outcomes in accordance with quality patient care and Company goals.
• Be familiar with emergency procedures and ensure regular attendance.
