Job summary

U.S. Renal Care in Monterey is seeking a Social Worker to join a cross-functional team and help patients living with kidney disease achieve maximum social functioning and psychological adjustment to dialysis treatment.

You will perform psychosocial assessments, coordinate care planning, provide education and referrals, advocate for patients and families, and collaborate with physicians, hospitals, and community resources to support optimal patient outcomes and compliance with regulations.

Qualifications

• Master's Degree in Social Work (CSWE-accredited) with LCSW preferred.
• Current state licensure required unless AZ/PA or Guam.
• Experience in dialysis social services preferred.
• Proficient in English, both written and spoken.
• Basic computer skills; MS Office; proficiency in USRC clinical applications within 90 days.
• Must meet state practice requirements.

Responsibilities

• Advocate for patients; perform psychosocial assessments and care planning.
• Coordinate education, referrals, and resources for patients and families.
• Collaborate with Medical Director, physicians, and community partners to ensure quality care and regulatory compliance.
