Job summary

Fresenius Medical Care seeks an entry-level Social Worker (MSW) to provide psychosocial services to in-center and home dialysis patients. You will work with the interdisciplinary team to promote rehabilitation, adjust to dialysis, and support families.

Responsibilities include care planning, counseling, transplant referrals, and coordinating with insurance and community resources. Prior licensure and a Master of Social Work are required; 0–2 years of related experience preferred.

Qualifications

• Masters in Social Work required and active state license.
• Meets scope-of-practice and licensure requirements in effect in the state of employment.

Responsibilities

• Assess patients' psychosocial status and plan care in collaboration with the interdisciplinary team.
• Provide counseling and education to patients and families to improve quality of life.
• Assist with transplant referrals and advance directive discussions with patients and families.
• Coordinate with Insurance Coordinators and community resources to address financial and support needs.
