Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current or successful CPR BLS Certification.
• Must meet the practice requirements in all states in which he or she is employed.

Responsibilities

• Performs all essential functions under the direction of the Supervisor and with guidance from the Educator, Preceptor or in collaboration with another Registered Nurse.
• Collects and analyzes patient data pre – during – post treatment and documents in the patient medical record, adjusts the treatment plan as indicated and notifies supervisor/physician/patient’s nurse as needed.
• Educates patient and family on basic learning needs and ongoing education.
• Reports patient status variations to the health team and collaborates for input.
• Directs and provides safe, effective care for assigned patients as prescribed for dialysis procedures.
• Assesses daily patient care needs and coordinates with staff as needed.
• Coordinates communication with dialysis providers and facility personnel for continuity of patient care.
• Administers medications as prescribed and documents justification and effectiveness.
• Participates in staff training and orientation; may provide instruction to hospital/facility staff.
