Job summary

Fresenius Medical Care is seeking an RN CAP 2 (Outpatient) to provide and coordinate high-quality hemodialysis care. You will assess, plan, implement, and evaluate care for an assigned patient group, while collaborating with RNs, LVNs/LPNs, and PCTs to ensure safe, effective dialysis therapy.

The role includes education for patients and families, potential precepting of new staff, and adherence to facility policies and quality initiatives.

Qualifications

• RN CAP 2 eligibility or CAP progression path.
• Minimum 3 months hemodialysis experience as RN.
• Current state RN licensure and CPR/BLS certification.

Responsibilities

• Provide and coordinate clinically competent care including assessment, planning, intervention, and evaluation for assigned patients.
• Delegate tasks to direct patient care staff (RNs, LVN/LPNs, PCTs) as appropriate.
• Administer medications per prescribed protocol or approved algorithms and document effectiveness.
• Educate patients and families about learning needs and care plans.
• Lead and mentor new staff as a preceptor or role model in the facility.
• Ensure correct lab collection, processing and documentation across patient care.
