Job summary

Grandresortok is seeking a Diabetes Care and Education Specialist to provide comprehensive diabetes self-management education and medical nutrition therapy in both individual and group settings. The role requires assessing needs, developing care plans, and promoting self-management to optimize health outcomes.

The DCES will document visits in the EHR, collaborate with the care team, and support technology-enabled education.

Qualifications

• Bachelor’s degree in nursing, dietetics/nutrition/pharmacy.
• Licensed in the State of Oklahoma.
• Experience working with patients with diabetes preferred.
• CDCES or willingness to receive certification within 24 months.

Responsibilities

• Delivers diabetes self-management education and medical nutrition therapy.
• Documents all contacts/visits in EHR and outcomes database.
• Collaborates with the diabetes care team on person-centered plans.
• Supports technology-enabled diabetes education and care.
