Job summary

Advocate Health Delavan is seeking a Diabetes Education Specialist or RN/RD with CDCES or potential for certification within 2–3 years to provide diabetes self-management education and support across inpatient and outpatient settings.

You will lead individual and group sessions on diabetes management, tailor interventions to patient needs, and collaborate with the healthcare team to implement comprehensive care plans. Strong communication and public speaking skills are essential.

Qualifications

• Current license or certification to practice as RN or RD in the applicable state.
• CDCES preferred; if not certified, certification within 2–3 years preferred.
• BSN preferred for nursing track; Master’s in Dietetics required if Dietitian license obtained after 01/01/2024.
• Excellent communication and time management skills.

Responsibilities

• Provides DSMES per ADA/ADCES standards.
• Assess learning needs, goals and barriers; tailor education.
• Lead individual and group diabetes education sessions (inpatient/outpatient).
• Provide medical nutrition therapy if RN is a Dietitian; follow care plans.
• Document and charge DSMT/MNT services per CMS/payor rules.
• Collaborate with care team; adjust medications per protocol when allowed.
• Maintain current knowledge of diabetes management tech and protocols.
• Stay compliant with ADA standards and organizational policies.

Education

RN or RD licensure

CDCES preferred

BSN preferred (if nursing)

Master's in Dietetics if licensure after 01/01/2024
