Stillwater Medical Group seeks a Diabetes Care and Education Specialist (RN or RDN) with advanced knowledge in diabetes care and education. The DCES will provide direct care, education, and support across the lifespan, helping individuals reach personalized nutrition, behavior, and treatment goals.

The role emphasizes evidence-based self-management, collaboration with clinicians, and adherence to confidentiality and professional standards.

Formación

• Bachelor's degree in nursing or nutrition with 2 years of diabetes care/education experience.
• Associate's degree in nursing with 4 years of diabetes care/education.
• License/registration: RN in MN or WI; or Dietetic registration (RDN).
• CDCES certification or ability to obtain within 24 months.
• Current BLS certification for health professionals.

Responsabilidades

• Provide direct care, education, and support across the lifespan for diabetes patients.
• Work with managing clinicians to deliver integrated, person-centered care.
• Lead diabetes education using evidence-based strategies and DSMES standards.
