Job summary

Regions Hospital seeks a Diabetes Care and Education Specialist (DCES) who can integrate RN or RD backgrounds to deliver diabetes education and care. The DCES provides direct care, education, and support across the lifespan, helping patients reach personalized nutrition, behavior, and treatment goals.

Using evidence-based strategies and collaborative care planning, the DCES promotes self-management, reduces risks, and improves health outcomes.

Qualifications

• Qualified candidates may be either a Registered Nurse or a Registered Dietitian Nutritionist with diabetes care and education experience.
• DCES role accepted in RN or RD backgrounds, with emphasis on education and self-management support.

Responsibilities

• Provide direct care, education, and support across the lifespan for people with diabetes.
• Promote self-management, reduce risks, and improve health outcomes through collaborative care planning.
• Apply a structured care approach: assess, plan, implement, evaluate, aligned with RN or Dietetic standards.
• Serve as a resource for the broader care team and ensure patient confidentiality and rights.
