Job summary

UC Health seeks a Diabetes Care and Education Specialist (DCES) to educate patients in ambulatory and inpatient settings, focusing on individualized care plans to support diabetes self-management. Qualifications

• Minimum required: Associate degree; preferred: Bachelor’s degree.
• CDCES certification required within 2 years of hire.
• RN track or Dietitian track options available with relevant licensure.

Responsibilities

• Engage in patient education and self-management support for diabetes.
• Provide DSMES with structured follow-up and device data review.
• Collaborate with physicians, nurses, dietitians, and pharmacists.
• Assist with discharge planning for inpatient care and community resources.
• Track outcomes and contribute to DES program quality and marketing efforts.
