Job summary

Advocate Health in Port Washington, WI, is seeking a Diabetes Care and Education Specialist to deliver DSMES and MNT under ADA/ADCES guidelines. You will lead individual and group education on diabetes management, collaborate with the care team, and adjust medications per protocol when approved by providers.

Requires RN or RD licensure; CDCES preferred; 2 years of patient education experience; BSN preferred; strong communication and public speaking skills. 30 hours/week with full-time benefits.

Qualifications

• Current license to practice as RN or Registered Dietitian in the applicable state; CDCES preferred.
• If not certified, certification must be obtained within 2–3 years preferred but not to exceed 3 years.
• 2 years of patient-oriented educational experience preferred with broad diabetes knowledge.

Responsibilities

• Lead individual and group diabetes education sessions (DSMES) including meds, nutrition, exercise, and glucose monitoring.
• Provide medical nutrition therapy for diabetes if RD and appropriate diagnosis.
• Collaborate with the care team to develop comprehensive care plans and follow CMS/payor guidelines.
