Job summary

Advocate Health seeks a diabetes education specialist at the Port Washington Clinic to lead DSMES and nutrition education for patients with diabetes. You will assess learning needs, tailor interventions, and conduct individual and group sessions focused on medication, nutrition, exercise, and glucose monitoring.

The role requires RN or RD licensure with CDCES preferred; BSN or Dietetics degree is expected, with ongoing certification supported.

Responsibilities

• Provides DSMES per ADA/ADCES standards.
• Assess learning needs, goals, and barriers for diabetes care.
• Lead individual and group education sessions on diabetes management.
• Provide medical nutrition therapy for diabetes when RD licensed.
• Collaborate with healthcare team to develop care plans.
• Stay current on ADA/CDCES standards and CMS guidelines.
• Document and charge for DSMT/MNT services.
