Job summary

HealthPartners is seeking a Diabetes Care Specialist and Education expert to provide direct patient care, education, and support across the lifespan. The role requires RN or RD credentials with advanced diabetes knowledge, delivering personalized nutrition and behavior guidance.

The DCES will use evidenced-based strategies, promote self-management, and work closely with clinicians to improve health outcomes. Strong communication and data-driven care planning are essential.

Qualifications

• Bachelor's degree in Nursing or Nutrition with diabetes care experience.
• Current licensure in the state or eligibility to obtain licensure.
• CDCES preferred or able to obtain within 24 months.

Responsibilities

• Assess patient needs, develop and implement a personalized care plan.
• Coordinate with diabetes team to adjust therapy per evidence-based protocols.
• Provide education on self-management and nutrition to patients and families.
• Document care and education in the EHR accurately and timely.
• Collaborate with clinicians and care teammates to ensure continuity of care.
