Job summary

Advocate Health is seeking a Diabetes Education Specialist to provide DSMES per ADA/ADCES standards to patients in inpatient and outpatient settings. The role includes assessing learning needs, leading individual and group sessions on diabetes management, and collaborating with the care team to implement comprehensive care plans.

RN or Registered Dietitian licensure is required; CDSC preferred. If not certified, certification should be pursued within 2–3 years.

Qualifications

• Current license to practice as RN or Registered Dietitian.
• If not certified, certification within two to three years preferred.
• Degree in Nursing, Dietetics, or related discipline meeting CDCES eligibility.
• BSN preferred if nursing degree; Masters in Dietetics required if Dietician license after 2024.

Responsibilities

• Provide care following the National Standard for Diabetes Self-Management Education and Support (DSMES) per ADA/ADCES.
• Assess patient learning needs, goals, and barriers; tailor educational interventions.
• Lead individual and group education sessions on diabetes management (medication, nutrition, exercise, glucose monitoring).
• Provide MNT services if a Registered Dietitian.
• Provide ongoing support and follow-up for diabetes management.
• Maintain knowledge of diabetes management technology and manufacturer protocols.
• Collaborate with the healthcare team to develop comprehensive care plans.
• Document and charge services per CMS/payor guidelines for DSMT and MNT.
