Job summary

Duly Health and Care is hiring a Dietitian to provide diabetes education for Type 1, Type 2 and Gestational Diabetes. You will also deliver Medical Nutrition Therapy to patients aged 2 through adulthood and document care in the EMR.

The role requires assessment of health history, risk factors and learning barriers, plus collaboration with the care team to support behavior change and educated decisions in daily life. Evening or weekend availability may be needed.

Qualifications

• Bachelor’s degree in Dietetics or equivalent is required.
• CDCES certification or active pursuit is required.
• Illinois Dietitian license and CDR registration are required.
• Current CPR status is required.

Responsibilities

• Provide diabetes self-management education to patients with Type 1, Type 2 and Gestational Diabetes.
• Provide Medical Nutrition Therapy to patients 2 years and older.
• Assess health history, risk factors, learning barriers and cultural factors to tailor education.
• Teach individuals and groups on diabetes self-management and nutrition.
• Document assessments, interventions and follow-up plans in the EMR.
• Collaborate with interdisciplinary team for diabetes care plans.
• Support patients in behavior change and self-management strategies.
