Job summary

Advocate HealthPort Washington clinic is seeking a 30-hour-per-week educator for diabetes education and medical nutrition therapy. You will lead DSMES sessions, assess patient needs, and collaborate with the care team to implement comprehensive diabetes management plans in outpatient and inpatient settings.

Eligibility requires RN or RD licensure with ADA/ADCES knowledge; CDCES preferred. Benefits, consistent schedule, and ongoing professional development support are included.

Qualifications

• Current license to practice as RN or RD in the applicable state.
• Certified Diabetes Care and Education Specialist (CDCES) preferred.
• If not certified, you must obtain certification within two to three years.
• Education: Degree in Nursing, Dietetics, or related discipline meeting CDCES eligibility.
• 2 years of patient-oriented educational experience preferred.

Responsibilities

• Provide care following the National Standard for DSMES per ADA/ADCES.
• Assess patient learning needs, goals, and barriers; tailor education accordingly.
• Lead individual and group education sessions on diabetes management.
