Job summary

Prince William County Government is seeking a temporary Service Coordinator (Clinical Services Caseworker) in the Developmental Disability Services Program. The role supports individuals with developmental disabilities, managing caseloads, conducting intakes, needs assessments, and service plan development.

You will link and coordinate services, monitor care, document interactions, and work in a team to meet documentation and productivity targets.

Qualifications

• Bachelor’s degree or higher in a human services-related field (e.g., sociology, social work, psychology)
• 2 years of experience providing service coordination/case management to individuals with developmental disabilities

Responsibilities

• Maintain a caseload of individuals needing or receiving services from community providers or other CS programs
• Provide case management including intakes, needs assessment, and service plan development
• Link, monitor, coordinate, and facilitate services needed by individuals
• Conduct assessments for individual service plan development
• Coordinate and facilitate treatment team meetings
• Implement and revise service plans as needed
• Monitor provision of services and assure plan adherence
• Document services, contacts, and attempted contacts with individuals and collaterals
