Job summary

Tri-Counties Regional Center is seeking a qualified service coordinator to provide person-centered consultation, advocacy, and resource information for individuals with developmental disabilities. You will participate in IDT meetings, coordinate IFSP/IPP plans, and connect clients with community resources and support services.

The role emphasizes collaboration with families and community agencies, along with strong planning, communication, and knowledge of public benefit programs.

Qualifications

• Minimum qualifications include Master’s or Bachelor’s in human services field with 2 years of applicable mental health or social work experience.
• 2 years experience in case management within a related field; 1 year experience with individuals with developmental disabilities preferred.
• Valid California driver’s license and reliable vehicle; ability to travel to any location.

Responsibilities

• Conduct strength-based assessments and involve individuals and families in person-centered planning and support activities.
• Design, document, implement and monitor services and supports in a timely manner.
• Develop and maintain a network of vendors and community agencies; provide information and training about developmental disabilities, services and resources.
