Job summary

Tri-Counties Regional Center is seeking a Service Coordinator II in Atascadero, CA to provide person-centered advocacy and comprehensive service coordination for individuals with developmental disabilities.

You will collaborate with a multidisciplinary team, prepare IFSPs and IPPs, attend IDT and IEP meetings, and connect families to community services and resources. A BA in Human Services and 2+ years of relevant experience are required, plus a CA driver’s license.

Qualifications

• Knowledge of developmental disabilities.
• Awareness of entitlement and public benefit programs.
• Ability to make verbal and written presentations to groups.
• Organizational and time management skills.
• Ability to work independently and within a team.
• Responsive to supervision.
• Adhere to applicable laws, regulations, policies and procedures.

Responsibilities

• Maintain regular contact with individuals served and develop IFSPs and IPPs.
• Attend IDT staffings and community meetings including IEPs and day program meetings.
• Provide information and referrals to community resources (schools, supports, day programs, agencies).
• Coordinate and secure TCRC funded services (behavior, independent living, supported living, respite).
• Develop and maintain a network of vendors and community agencies.
• Provide training about developmental disabilities and resources.
