Tender Loving Care of Duluth seeks a Client Care Manager to coordinate services for an assigned group of clients and guide direct support staff, turning person-centered plans into daily supports.

This full-time role emphasizes data-driven outcomes, strong communication, and a small locally-owned setting where thoughtful follow-through makes a visible difference. Schedule is Mon-Fri 8:00am–4:30pm with pay range shown.

Responsibilities

• Coordinate the delivery and evaluation of services for assigned clients.
• Oversee support plans and addenda, and help clients progress toward outcomes.
• Translate goals into practical service strategies and measurable outcomes.
• Instruct and coach direct support staff in implementing plans and reviewing competency.
• Evaluate services and suggest adjustments as needed.
• Act as main TLC contact for assigned clients and communicate with families, case managers, and partners.
• Complete accurate, timely service documentation and reports.
• Protect clients' health, safety, rights, dignity, confidentiality, and boundaries.
• Support staff orientation, training, and competency verification.
• Assist for licensing reviews and maintain 245D knowledge.
• Provide direct client support and participate in on-call coverage.
