Job summary

IMPaCT Care in Denver, CO is seeking a Community Health Program Coordinator to operationally backbone CHW teams, coordinating activities, data collection, and cross-site program operations.

You will support 1-2 Program Managers, schedule trainings, maintain documentation, analyze outcomes, and engage with community members to ensure program quality. Travel to multiple locations is required; no remote work.

Qualifications

• 3-5 years in program coordination, research administration, or equivalent experience.
• Strong writing and communication skills.
• Proven experience with data collection, analysis, and protocol implementation.
• Proficient in Microsoft Word and Excel.
• Valid driver's license, must own a and have car insurance.
• Ability to travel to multiple locations across the Atlanta area.
• Bachelor's degree preferred.
• Ability to pass background check and pre-employment screening.

Responsibilities

• Support 1-2 Program Managers overseeing CHW teams serving hundreds of patients.
• Schedule training sessions, team meetings, and community events.
• Maintain program documentation, client records, and compliance requirements.
• Collect, organize, and analyze data to measure outcomes and improve program quality.
• Engage with community members, colleagues, and partners, including those who may be upset.
• Will be responsible for enrollment of patients into the program.
