IMPaCT Care is seeking a Manager of Community Health Worker Services in West Los Angeles, CA. You will lead a team of CHWs serving hundreds of clients annually, partnering with Kaiser Permanente and local organizations, and using data to improve outcomes and resource connections.

The role requires experience in non-profit settings, public health, social work, or case management, plus strong leadership and communication skills. A college degree is required; MSW/MPH is preferred.

Qualifications

• Leadership experience managing teams in non-profit or community settings.
• Extensive CHW, public health, social work, or case management background.
• Deep understanding of healthcare and social services systems and public benefits enrollment.
• Knowledge of local community resources and organizations.
• Comfort with analyzing data and using reports to drive performance.
• Excellent oral and written communication skills; proficient with email and mobile devices.
• Ability to travel to multiple locations; willingness to cover West LA and surrounding areas.
• College degree required; MSW or MPH preferred; background check and pre-employment screening.

Responsabilités
