Job summary

Penn Medicine in Philadelphia seeks a Program Assistant to support Community Health Worker (CHW) teams embedded in inpatient and outpatient settings, enrolling patients for the IMPaCT intervention and assisting with quality improvement initiatives.

You will use the electronic health record to identify eligible patients, support health-related goal setting with CHWs, and ensure accurate documentation in REDCap and other platforms.

Qualifications

• H.S. Diploma/GED is required.
• 1–2 years experience in health care, non-profit, or community-based organizations, and service in low-income communities.
• Bachelor of Arts or Science is required.
• Health Sciences, Social Work, Sociology, or Public Health background preferred.

Responsibilities

• Identifies, outreaches, and enrolls patients eligible for the IMPaCT intervention, ensuring program enrollment goals are met weekly and annually.
• Uses the electronic health record to identify and produce reports of patients meeting eligibility for CHW support; supports documentation in REDCap and data reporting.
