Qualifications

• High school diploma or GED and 3–6 months related experience preferred.
• Knowledge of dementia, particularly Alzheimer's type.
• Refer to state regulations for medication assistance where required.

Responsabilités

• Provides dementia care including adapted ADLs, meaningful engagement, and person-centered service planning.
• Assists residents with daily living activities and supports independence as much as possible.
• Participates in team discussions and understands residents' Life Story, preferences, and routines.
• Serves meals, supports nutrition, and documents changes in eating habits as needed.
• Maintains resident records daily and reports health or behavioral changes to the supervisor.
• Follows medication procedures per state regulations and physician orders when permitted.
• Maintains a clean, safe, and orderly environment and follows emergency procedures.
