Qualifications

• High school diploma or GED; 3–6 months direct care experience preferred.
• Knowledge of dementia care preferred.
• Ability to follow care plans and communicate in English.

Responsibilities

• Provide dementia care with meaningful engagement per individual care plans.
• Assist with ADLs, mobility, meals, and daily living activities as assigned.
• Maintain accurate records and monitor health/emotional condition.
• Support teamwork and communicate with residents, families, and staff.
• Follow safety procedures and report changes to supervisor.
